What is the Chronic Conditions Prevention and Integrated Care Program (CCPIC)?
The Chronic Conditions Prevention and Integrated Care Program is a national competitive grants program supporting initiatives that improve health outcomes for Australians living with, or at risk of, chronic conditions.
With a total funding allocation of $148.235 million over four years, the program strongly prioritises collaborative, evidence-based and sustainable projects that improve health equity and strengthen integrated care systems.
Chronic Conditions Prevention and Integrated Care Program level of support
There are two funding tiers available under the program.
Tier 1 โ National Collaborative Activities
Tier 1 supports large-scale national initiatives that:
- address multiple chronic conditions and/or risk factors
- align with at least two program objectives
- demonstrate national reach
- involve cross-sector collaboration
- deliver scalable and sustainable outcomes.
Funding Available
- Minimum funding: $1.25 million per annum
- Maximum funding: $3 million per annum
- Maximum total funding: Up to $12 million
- Grant duration: Up to 3.5 years
Co-contribution Requirement
- No mandatory co-contribution requirement is specified in the guidelines.
- However, applicants are encouraged to demonstrate value for money, sustainability, partnerships, and in-kind contributions where possible.
Approximately 70% of total funding is expected to support Tier 1 activities.
Tier 2 โ Targeted Activities
Tier 2 supports:
- targeted interventions
- pilot projects
- time-limited trials
- single-condition initiatives
- smaller-scale prevention or integrated care activities.
Funding Available
- Minimum funding: $375,000 per annum
- Maximum funding: $1.25 million per annum
- Maximum total funding: Up to $2.5 million
- Grant duration: Up to 1.5 years
Co-contribution Requirement
- No mandatory co-contribution requirement is specified in the guidelines.
- Applicants should still demonstrate strong value for money and sustainable delivery.
Approximately 30% of total funding is expected to support Tier 2 activities.

2. Embedding Prevention and Early Detection
Supporting:
- prevention strategies
- screening
- early diagnosis
- early intervention models
- targeted prevention initiatives.
3. System Integrated, Multidisciplinary and Person-Centred Continuity of Care
Improving:
- care coordination
- continuity of care
- multidisciplinary collaboration
- digital health integration
- integrated service delivery across life stages and health systems.
4. Managing Multimorbidity
Supporting integrated and person-centred approaches that reduce fragmentation for individuals living with multiple chronic conditions.
5. Enhanced and Targeted Support for Health Equity
Delivering tailored support for:
- Aboriginal and Torres Strait Islander peoples
- culturally and linguistically diverse communities
- people living in rural and remote areas
- people experiencing socioeconomic disadvantage
- people living with disability
- other priority populations experiencing health inequities.
Projects and expenditure eligible for Chronic Conditions Prevention and Integrated Care Program support
Eligible projects may include:
- systemic reform initiatives aligned with the National Strategic Framework for Chronic Conditions 2026โ2035
- prevention and early intervention activities
- multidisciplinary care coordination initiatives
- clinician education and workforce capability projects
- consumer awareness and education campaigns
- development or updating of clinical guidelines and standards of care
- community engagement and co-design activities
- stakeholder collaboration and partnership projects
- targeted chronic condition health services addressing identified service gaps
- data collection and analysis activities
- culturally safe support initiatives for priority populations
- workforce development and training strategies
- integrated digital health or care coordination initiatives.
Ineligible Expenditures
Grant funding cannot be used for:
- purchase of land
- major capital works or construction
- retrospective costs
- overseas travel
- chartered flights
- core peak body activities
- general organisational administration costs unrelated to the project
- ongoing operational costs not directly linked to delivery
- preparation of grant applications
- ransomware or cybercrime payments
- purely research-focused projects
- clinical trials
- activities already funded by another government source
- activities primarily focused on: cancer, mental health, alcohol and other drugs, and palliative care.
The program also excludes activities considered the responsibility of other existing government systems such as:
- MBS
- NDIS
- NHRA funded services.

Organisations eligible for Chronic Conditions Prevention and Integrated Care Program support
Eligible applicants include:
- Aboriginal and/or Torres Strait Islander Corporations
- companies
- cooperatives
- incorporated associations
- statutory entities
- partnerships
- non-government organisations
- Primary Health Networks
- community organisations
- academic institutions.
Consortia applications are encouraged.
Applications submitted by consortia must:
- nominate one eligible lead organisation
- include letters of support
- clearly define partner roles and responsibilities.
Chronic Conditions Prevention and Integrated Care Program assessment criteria
Applications will be assessed against four criteria.
Applicants must score at least an โAverageโ rating against every applicable criterion to remain competitive.
Assessment Criterion 1 โ Alignment with Grant Objectives and Outcomes
Weighting
- 40% for standard activities
- 30% for health equity-focused activities.
Applicants must demonstrate:
- alignment with relevant program objectives
- alignment with the selected funding tier
- evidence of project need
- relevant data, evidence or scenarios supporting the activity
- stakeholder collaboration strategies
- evaluation methodology
- project delivery locations
- contribution to Closing the Gap outcomes where relevant
- national delivery approach for Tier 1 projects.
Applicants must also submit an Activity Work Plan detailing:
- project tasks
- milestones
- deliverables
- outputs
- performance measures
- evaluation methods
- implementation timelines.
Assessment Criterion 2 โ Organisational Capacity and Capability
Weighting
- 30%
Applicants must demonstrate:
- proven experience delivering similar projects
- access to appropriately skilled personnel
- effective project governance
- project planning capability
- milestone and risk management systems
- stakeholder and partnership management capability
- infrastructure and delivery methodology
- co-design experience with priority populations
- culturally safe delivery capability.
Applicants must also submit a comprehensive Risk Management Plan covering:
- identified risks
- likelihood of occurrence
- impact assessments
- mitigation strategies.
Assessment Criterion 3 โ Efficient and Economical Use of Grant Funds
Weighting
- 30%
Applicants must demonstrate:
- value for money
- cost-effective delivery
- financial sustainability
- ethical use of public funding
- realistic and appropriate budgets
- efficient allocation of funding
- high-quality outcomes relative to investment.
Applicants must submit an Indicative Activity Budget outlining:
- expenditure breakdowns
- in-kind contributions
- consortium allocations
- intended outcomes linked to funding.
Assessment Criterion 4 โ Enhanced and Targeted Support for Health Equity
Weighting
- 10%
- Only applies to projects targeting priority populations.
Applicants must demonstrate:
- how the activity addresses service gaps for priority populations
- evidence supporting the proposed intervention
- culturally safe project design
- co-design and consultation capability
- policies and procedures supporting health equity
- how intersectionality impacts are considered
- tailored service delivery approaches.
Priority populations include:
- Aboriginal and Torres Strait Islander peoples
- CALD communities
- people living in rural and remote areas
- people with disability
- people experiencing socioeconomic disadvantage
- people with mental illness.

To improve the likelihood of receiving funding, applicants should demonstrate:
